Indian Health Service, Aberdeen Area Office reported to HHS on November 13, 2014, an incident where an employee of a business associate sent protected health information to a personal email account. The breach, which occurred between March 2013 and August 2014, affected 1,632 individuals and involved names, Social Security numbers, and dates of birth. In response, the entity terminated the employee's contract and obtained written assurance that the data was deleted.
