Breached PHI included names, addresses, dates of birth, Social Security numbers, financial and health insurance information, diagnoses, medications, and other treatment information. Breached information was located on Email systems. The CE notified individuals, the media, and provided substitute notice; credit monitoring was offered and staff were retrained on email security.
