Breached information was located on Electronic Medical Record and Network Server systems. PHI involved included names, addresses, dates of birth, drivers' license information, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, and the media, implemented additional safeguards, and revised HIPAA policies and procedures following an OCR investigation.
