InterMed, PA (Maine healthcare provider) reported to HHS OCR on 2020-01-08 a Hacking/IT Incident affecting approximately 33,000 individuals. An employee was the victim of an email phishing scheme. PHI involved included names, birthdates, health insurance information, clinical information, and Social Security numbers. Breached information located in Email. The CE notified HHS, affected individuals, and the media, and subsequently implemented additional administrative and technical safeguards and retrained its workforce.

InterMed, P.A. notified patients that an unauthorized individual accessed an employee's email account between September 4-6, 2019. The incident involved phishing leading to credential compromise. Patient data including name, DOB, health insurance, and clinical info may have been viewed. No misuse believed.
