Andrew Dang, DDS reported a physical burglary of its Monterey Park office on April 19, 2020, during which an office computer was stolen. The unencrypted computer contained patients' and employees' social security numbers and dental insurance information. The practice reported the incident to police and regulators, and is offering two years of complimentary identity theft protection to affected individuals. The practice is currently encrypting patient information to prevent future incidents.

The CE notified HHS, affected individuals, and the media, and provided free credit monitoring. Following the breach, the CE improved physical safeguards and implemented additional technical safeguards. OCR obtained assurances that corrective actions were implemented.
