PHI involved included names, dates of birth, Social Security numbers, addresses, financial information, driver's license/state ID numbers, diagnoses, lab results, medications, and health insurance/treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Complimentary credit monitoring was offered; additional safeguards were implemented and staff were retrained.
