NYU Hospital for Joint Diseases Inventory Management Department reported to HHS on 2011-09-26 a Theft affecting 2,600 individuals. A box of 2,600 paper tissue-implant records was discarded by a waste disposal contractor when not properly secured; documents were buried in an out-of-state landfill. PHI of 2,239 individuals included names, dates of birth, surgery dates, surgeon names, procedures, and tissue serial numbers. Breached information located on Paper/Films. OCR investigation resulted in locked storage, staff retraining, and a HIPAA compliance review plan.
