Several employees were victims of an email phishing scheme exposing ePHI located in Email systems. Breached data included names, addresses, dates of birth, driver's license numbers, Social Security numbers, claims and financial information, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, affected individuals, and media, and retrained staff on phishing identification.
