PHI exposed on Email systems included names, dates of birth, addresses, driver's license numbers, SSNs, phone numbers, email addresses, passport numbers, state/military/tax ID numbers, financial information, diagnoses, prescription information, and health insurance/treatment information. The CE notified HHS, affected individuals, and the media. The CE subsequently ceased operations and is no longer in business.
