Arthritis & Osteoporosis Consultants of the Carolinas (NC) reported to HHS on 2018-11-09 an Improper Disposal breach affecting 3,930 individuals. On September 10, 2018, an employee disposed of PHI in a trash can instead of a shred bin. Exposed information included patients' names, dates of birth, insurance information, treating physicians, infusion drug names (some patients), and Social Security numbers (Medicare patients). Breached information located on Paper/Films. The CE sanctioned and retrained the responsible clerk, added shred bins, and reviewed PHI disposal policies.
