BlueCross BlueShield of Western New York

BlueCross BlueShield of Western New York (acting as a Business Associate) reported to HHS on 2013-01-22 a Theft/Loss incident affecting 725 individuals. The BA mailed monthly premium invoices containing PHI (names, member ID numbers, SSNs) that were never received by the covered entity. Breached information was located on Paper/Films. The BA contacted the U.S. Post Office, notified affected individuals, and revised invoice procedures to remove sensitive identifiers and send invoices via secure email. OCR verified a proper BA agreement was in place.
