Per the web description, the business associate Physician Insurance A Mutual Company reported an employee fell victim to an email phishing scheme, compromising PHI including names, Social Security numbers, dates of birth, health insurance information, and treatment information. Breached information located on Network Server. The BA notified HHS, affected individuals, and media, and provided credit monitoring and new administrative/technical safeguards.
