The breach affected approximately one New Hampshire resident, involving names and Social Security numbers. Discovery occurred on April 19, 2024. Notifications began May 9, 2024, offering 36 months of credit monitoring.

Texas Panhandle Centers (TX) reported to HHS OCR on 2024-05-09 a ransomware incident affecting 16,394 individuals via a Network Server. Exposed PHI included names, dates of birth, Social Security numbers, addresses, drivers' license numbers, claims information, diagnoses and conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media, and implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
