Assisted Living Concepts notified employees that a payroll vendor was breached between Dec 14, 2013 and Jan 14, 2014. An unauthorized third party obtained vendor credentials and accessed payroll files containing names, addresses, birthdates, SSNs, and pay info. The company reported the incident to the FBI and IRS, deactivated compromised credentials, took systems offline, and offered one year of Experian ProtectMyID. The investigation was ongoing as of the notice date.
