Centerstone International Fund

Centerstone reported to HHS on July 2, 2010, a breach affecting 1,537 individuals due to a major flooding event that damaged a building housing its school-based program offices. The flood, which led to the area being declared a federal disaster zone, resulted in the loss of paper records and water damage to desktop computers. The compromised Protected Health Information (PHI) included names, addresses, dates of birth, and Social Security numbers.
