Mental Health Center for Southern New Hampshire Inc

Seacoast Mental Health Center (SMHC) detected unusual activity on an email account on November 30, 2022. A forensic investigation concluded on January 18, 2023, that two email accounts were compromised by an unauthorized actor. 426 New Hampshire residents were potentially affected. SMHC secured accounts, engaged forensics, reset credentials, and offered credit monitoring. No evidence of misuse was found.

CLM Center for Life Management, a non-profit mental health provider, notified 27 New Hampshire residents of a security incident where an unauthorized person accessed their court diversion program system. The attacker moved and deleted data but did not exfiltrate personal information. Potentially viewed data included contact info, DOB, SSN (for 22 individuals), diagnosis codes, and medication names. CLM engaged forensic investigators and offered 1 year of credit monitoring.

Center for Mental Health notified Montana residents that a document containing names, SSNs, DOBs, and limited health info was lost by an employee during a medication delivery in Nov 2016. The document was found by a child and returned to police. The company offered 12 months of identity protection.
