A business associate mislabeled packages containing lancet devices, causing shipping labels to display the wrong member's name, implying the individual had diabetes. The CE terminated the BA relationship and implemented a quality assurance process.

Livongo Health, Inc. notified Montana residents of a misdelivery incident involving a third-party vendor. On November 26, 2015, a vendor shipped lancing devices to Livongo members but mislabeled packages, sending one recipient's name on a package delivered to another member. Only the affected individuals' names were disclosed. Livongo discovered the error on November 28, 2015, terminated the vendor, and is reviewing protocols.
