The City of Chicago reported that due to a misconfiguration, protected health information for approximately 2,080 students was unintentionally exposed on the internet. The exposed data included names, birthdates, diagnoses, and other details. The entity worked with Google to remove the cached information and notified affected individuals.

City of Chicago notified New Hampshire AG regarding a stolen laptop belonging to ambulance billing contractor DeZonia Group. The theft occurred on January 23, 2009. The laptop contained names and Social Security numbers of NH residents. The device was password-protected and encrypted.
