UAW Retiree Medical Benefits Trust (Health Plan, MI) reported to HHS OCR on 2021-12-14 a Hacking/IT Incident affecting 576 individuals. A vendor of its business associate was the victim of a ransomware attack that affected ePHI stored on a Network Server. ePHI involved included names, addresses, dates of birth, member ID numbers, diagnoses, medications prescribed, and other treatment information. The case was consolidated into an ongoing investigation.
