Motorola Solutions (Health Plan, IL) reported to HHS OCR on 2025-11-05 a Hacking/IT Incident affecting 22,600 individuals. PHI was located on a Network Server. The covered entity's business associate experienced a cyberattack exposing clinical, demographic, and financial PHI.
