Motorola Solutions (Health Plan, IL) reported to HHS OCR on 2025-11-05 a Hacking/IT Incident affecting 22,600 individuals. PHI was located on a Network Server. The covered entity's business associate experienced a cyberattack exposing clinical, demographic, and financial PHI. In response, Motorola Solutions provided credit monitoring to affected individuals and both the CE and BA implemented additional administrative, technical, and security safeguards.

Motorola's Enterprise Mobility Solutions (Symbol) notified the NH Attorney General on Feb 6, 2009, that a security vulnerability on the software.symbol.com e-commerce site (operational 2000-2005) allowed unauthorized access to personal data of 6 NH residents. Data included names, contact info, and payment card numbers (mostly expired). Motorola worked with a third-party host to investigate, remediate, and notify affected individuals.
