Blue Cross & Blue Shield of Rhode Island reported to HHS on 2018-09-13 an Unauthorized Access/Disclosure affecting 1,567 individuals. Breached information was located on Paper/Films. Business associate RedCard, a mailing services provider, improperly consolidated Explanation of Benefits (EOB) documents, causing EOBs for different individuals at the same address to be mailed together. Exposed PHI included names, plan ID numbers, provider names, types of medical service, and claim information. The BA ceased consolidating EOBs. OCR found the BA agreement compliant with the Privacy Rule.

Blue Cross & Blue Shield of Rhode Island reported to HHS on 2010-04-21 a theft affecting 12,000 individuals. The breach occurred when the company donated a file cabinet containing paper records with protected health information (PHI) before it was properly cleaned out. The compromised data included names, addresses, phone numbers, Social Security numbers, and Medicare ID numbers. In response, the company offered one year of free credit monitoring, retrained staff, and revised its policies.

On January 5, 2010, Blue Cross Blue Shield of RI (BCBSRI) was notified that a 16-page report pertaining to Brown University's health plan was impermissibly disclosed to two other BCBSRI agents. The report contained the protected health information of approximately 528 individuals, including first and last names, dates of service, cost of medical care, and member identification numbers. In response, BCBSRI recovered the reports, received assurances that electronic copies were deleted, notified affected individuals, implemented new procedures for correspondence, and began auditing affected members' claim histories.
