A former employee sent ePHI — including names, birthdates, addresses, diagnoses, and treatment information — to her personal and a family member's email accounts. The CE notified HHS, affected individuals, and the media, and implemented additional administrative/security safeguards and staff retraining. OCR obtained assurances of corrective action.
