Sharp HealthCare reported to HHS on 2025-06-06 a Hacking/IT Incident affecting 500 individuals. Breached information located on Network Server.

Episource, LLC disclosed a data breach affecting its computer systems. Unauthorized access occurred between January 27, 2025, and February 6, 2025, when a criminal viewed and copied data. The incident was discovered on February 6, 2025. Affected data includes contact information, health insurance data, health data (diagnoses, treatments), and other personal data such as driver's license numbers. Episource engaged forensic investigators, notified law enforcement, and shut down systems. They are offering two years of credit monitoring.

Sharp Healthcare reported to HHS on 2023-02-17 a Hacking/IT Incident affecting 62,777 individuals. Breached information located on Network Server. The PHI involved included names and financial and treatment information. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards.

On January 12, 2023, Sharp HealthCare identified suspicious activity on a server running its website. An unauthorized person gained access for a few hours and obtained a file containing patient names, internal identification/invoice numbers, payment amounts, and proof of payment to Sharp hospitals or medical groups. No SSNs, bank account details, credit card numbers, or clinical health information were involved. Sharp took servers offline, engaged a forensic firm, and enhanced website server security.

Sharp HealthCare reported the theft of a computer and external memory device from a secure storage area on February 6, 2017. The devices contained protected health information related to a blood pressure study, including names, dates of birth, ages, medications, and family history. Law enforcement is investigating. No misuse of data has been identified.
