Omnicell, Inc. (a business associate of covered entity Sentara) reported to HHS on 2012-12-31 a Theft affecting 56,820 individuals. An electronic medication dispensing device was stolen from the locked car of an Omnicell employee. Breached PHI included patient names, birth dates, patient numbers, medical record numbers, and clinical information. Breach notification was provided to HHS, the media, and affected individuals.
