Packaging Corporation of America (Health Plan, IL) reported to HHS on 2024-08-15 a Hacking/IT Incident affecting 783 individuals. A vendor of its business associate experienced a cyber-attack that compromised PHI including names, addresses, dates of birth, Social Security numbers, account identifiers, and other identifiers stored on a Network Server. The CE notified HHS and affected individuals, and provided complimentary credit monitoring. The BA and vendor are under investigation.
