On October 1, 2014, archived prescription dispensing logs and waived lab test logs left in an unlocked closet were mistakenly placed in a trash dumpster by a custodian and subsequently taken to a landfill. PHI exposed included names, dates of birth, lab results, and medications (Paper/Films). The CE later assessed the incident as non-reportable but took corrective action including staff retraining, HIPAA audits, and new bulk trash removal policies. OCR obtained written assurances.
