Marin Healthcare District reported to HHS on September 27, 2016, a hacking incident affecting 2,292 individuals. The incident involved a ransomware attack on the systems of its business associate, Marin Medical Practice Concepts, Inc. Although a forensic investigation found no evidence that patient information was accessed or transferred, a backup system failed during the restoration process. This failure resulted in the loss of protected health information created between July 11, 2016, and July 26, 2016, including vital signs, clinical histories, and physician-patient communications.
