The error was an isolated incident involving an employee sending an unencrypted email to an unintended third-party recipient. HealthMarkets investigated, coached the employee, contacted the recipient to destroy the data, and reviewed distribution practices. Notices were mailed in May 2013, and 12 months of credit monitoring was offered.
