CorrectionPublished Oct 11, 2026

The document Washington posted for this filing holds two HHS breach reports. This record was first built from a 2018 report (a check image texted to a wrong number, one person). The filing Washington lists, dated August 5, 2016, is the second: paper records of 12 hospice patients stolen in a break-in on June 13, 2016. We corrected the record to that notice.
