An unauthorized fraudulent account was used to access a server, downloading patient PII (names, addresses, DOB, SSNs, insurance info) of 75 New Hampshire residents. No financial or diagnosis data was compromised. Cornerstone isolated the server, restored systems from backups, and offered 12 months of credit monitoring.
