Quantum Health, a business associate of NTT Health Plan, notified the Plan on December 29, 2025, of an inadvertent disclosure of PII and PHI occurring between December 15 and December 29, 2025. A claims report containing names, addresses, SSNs, member IDs, and claim information was inadvertently disclosed. The recipient deleted the report. Quantum Health implemented additional safeguards and training. Affected individuals are offered 24 months of Experian IdentityWorks.
