Seneca Family of Agencies

Seneca Family of Agencies reported to HHS on 2022-01-22 a Hacking/IT Incident affecting 1528 individuals. Breached information located on Network Server. The covered entity experienced a cyber-attack affecting the protected health information (PHI) of 1,528 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, phone numbers, email addresses, medical record numbers, diagnoses, health insurance information, prescription information, drivers’ license/state identification numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards.
