On June 15 and June 19, 2015, an unauthorized individual broke the lock on a portable on-demand (POD) storage container and removed it, exposing PHI including names, addresses, dates of birth, SSNs, lab results, diagnoses, and clinical information. Breached information was located in the physical 'Other' storage medium. The CE notified affected individuals, HHS, and media; reported to local police; enhanced physical safeguards; and retrained staff on HIPAA policies. OCR confirmed corrective actions were implemented.
