Clinic of North Texas LLP discovered on November 9, 2021 that it was the victim of a cyber attack that may have resulted in unauthorized access to patient information stored on its systems. The breach occurred on or about October 4, 2021. Affected data included patient names, addresses, dates of birth, and limited health information. No SSNs, financial, or government ID data were compromised. Patients were notified March 18, 2022 and offered 12 months of complimentary credit monitoring through IDX.
