An employee removed a computer hard drive containing PHI — including patients' names, addresses, dates of birth, medications, treatment information, and medical record numbers — and sold it on the internet. The CE notified the Morris County NJ Prosecutor's Office and FBI, terminated the employee, revised electronic-asset management policies, improved physical security, and implemented new hard drive decommissioning procedures.
