The California Department of Rehabilitation experienced a data security incident on January 9, 2019, when a spreadsheet containing employee names, Social Security numbers, and classification information was saved to an internal network folder (G drive) accessible to DOR employees. The error was discovered on January 14, 2019, when an employee accessed the file and reported the presence of SSNs. The file was immediately deleted, and folder access was restricted. The Department notified affected employees on January 24, 2019, and offered 12 months of credit monitoring. No evidence of misuse was found.
