Deposition Solutions, LLC (d/b/a Lexitas) experienced a data breach due to a web server configuration error that occurred in or around September 2019. The error allowed certain files in its OnDemand application to be accessible over the internet without authentication. Lexitas discovered the issue on May 29, 2026, after being notified by a customer. The incident potentially affected approximately 14,361 individuals nationwide, exposing personal information including names, Social Security numbers, driver's license numbers, financial account information, dates of birth, and medical/health insurance information. Lexitas restricted access to the server within hours of discovery and engaged a forensic firm. No evidence of malware or active threat actor presence was found. Affected individuals are offered credit monitoring and fraud assistance services.
