Tower Imaging, LLC (FL) reported to HHS on 2019-04-16 the theft of an unencrypted laptop from its medical facility, discovered on February 19, 2019. The laptop, which was not recovered, contained PHI for 810 patients including names, dates of birth, and medical record numbers. In response, the CE retrained staff, updated policies, reviewed facility security codes, performed a risk analysis, adopted a risk management plan, purchased an encrypted replacement laptop, and locked it to a rolling cart. OCR obtained assurances of corrective action implementation.
