PHI included names, dates of birth, medical record numbers, ultrasound information, exam dates, and reasons for the ultrasound. The computer required proprietary software and an electronic key to access PHI. The CE notified affected individuals, HHS, and media, cooperated with law enforcement, and upgraded facility access controls with proximity card readers. OCR investigation prompted updated risk analysis and additional risk management.
