An unauthorized individual accessed the entity's server, compromising demographic, clinical, and healthcare payment information stored in the Electronic Medical Record and Network Server. In response, the entity removed the server from the network, disabled the unauthorized account, implemented software patches, and encrypted its wireless network and email. OCR provided technical assistance.
