CareMeridian, LLC discovered on December 23, 2017, that an unencrypted disk sent by third-party software provider Bullpen Financial, Inc. was lost in the mail. The disk contained protected health information (PHI) and names of 953 California residents, and names and Social Security numbers of 13 California residents. CareMeridian engaged forensic experts, ceased business with the vendor, and provided credit monitoring to those with SSNs exposed. Notifications were sent in March 2018.
