Wake Family Eye Care (NC) reported to HHS on 2023-07-14 a ransomware incident affecting 14,221 individuals. PHI stored on a network server was compromised, including names, dates of birth, addresses, Social Security and driver's license numbers, health insurance information, optical images, and other treatment information. The CE notified HHS, affected individuals, and media, posted substitute notice, offered free credit monitoring, and implemented additional safeguards. Staff were retrained on PHI protection requirements.
