UNITED SEATING AND MOBILITY, L.L.C.

On April 14, 2015, two unencrypted tablet computers, a smartphone, and a backpack containing paper files were stolen from two company vehicles belonging to National Seating & Mobility, Inc. (TN). The breach affected 9,627 individuals and involved demographic, clinical, and financial PHI. The CE notified HHS, affected individuals, and media, posted substitute notice, revised policies, encrypted portable devices, and deployed remote wipe/tracking. OCR obtained corrective action assurances.
