FISHMAN TRANSDUCERS INC

Fishman Vision, an ophthalmology clinic in Palo Alto, CA, notified patients that their data may have been exposed due to a security incident at their vendor, Eye Care Leaders (ECL), in December 2021. An unauthorized party accessed ECL's 'myCare Integrity' platform and deleted databases. While no evidence suggests Fishman Vision's specific patient records were accessed, PHI and PII (including SSNs) may have been stored on the system. Fishman Vision terminated its relationship with ECL and offered 12 months of identity monitoring via Kroll.

Fishman Vision reported to HHS on 2022-06-01 a Hacking/IT Incident affecting 2646 individuals. Breached information located on Electronic Medical Record. The breach involved a business associate cyberattack exposing PHI including names, addresses, DOB, SSNs, and diagnoses. Response included credit monitoring.
