Compromised data included names, addresses, dates of birth, driver's license numbers, Social Security numbers, diagnoses, claims and financial information, medications, and other treatment information. The hospital notified HHS, affected individuals, and the media, and implemented additional administrative, technical, and security safeguards. No business associate was reported as involved.
