Synergy Healthcare Services, a healthcare organization, reported a data breach to the Maine Attorney General. The breach, described as an external system breach (hacking), occurred between November 30, 2022, and December 3, 2022, and was discovered on May 16, 2023. The breach affected 4 Maine residents, and the compromised information included names or other personal identifiers in combination with financial account numbers or credit/debit card numbers. Synergy Healthcare Services offered one year of credit monitoring services through Experian to affected individuals.

Synergy Healthcare Services (GA), a HIPAA Business Associate, reported a ransomware attack affecting PHI of 296,956 individuals. PHI included names, DOB, addresses, driver's license and SSNs, diagnoses, medications, lab results, financial/claims information, and other treatment information. Notifications were sent to HHS, affected individuals, and the media; substitute notice was provided. Credit monitoring was offered and additional administrative, technical, and security safeguards were implemented.

Synergy Healthcare Services notified consumers of a December 2022 unauthorized network access incident affecting PHI and PII (SSN, DL, bank accounts) of patients from affiliated care centers. The breach was discovered in early December 2022, investigated by third-party experts, and notification was sent in July 2023. No fraudulent use suspected. Credit monitoring offered.

Synergy Healthcare Services reported a data breach to the Montana Attorney General. The breach was reported on 2023-07-26. The breach occurred from 12/1/2022 to 12/31/2022. 24 Montana residents were affected.
